This test stimulates your acoustic nerve by delivering cold or warm water or air into your ear canal. When cold water or air enters your ear and the inner ear changes temperature, it should cause fast, side-to-side eye movements called nystagmus.

Traditionally, the caloric test is performed with the patient lying with the head elevated 30°. Cold (30°C) and warm (44°C) water are used to irrigate one ear at a time. Cold irrigation is an inhibitory stimulus, and warm irrigation is excitatory.

How long does caloric test take?
Nystagmus commonly builds for about 30-60 seconds, then gradually decays away over roughly 2 minutes. After a rest of at least 5 minutes, the procedure is repeated with either the opposite temperature water, or on the other side.
